Community Health Centre
The Health Centre provides health promotion and preventive services across the lifespan for community members of Munsee Delaware Nation. The department focuses on immunizations, health education, and prevention initiatives related to maternal health, chronic disease prevention, communicable diseases, diabetes management, and other priority health areas. Services are delivered using a culturally safe, community-based approach to support the holistic health and wellbeing of the Indigenous community.

TAKING ACTION
Objectives
- Develop and deliver health education programs in collaboration with Health and Wellness workers to address community-identified health priorities.
- Provide home visits for clients following hospitalization or injury to complete assessments, develop care plans, provide referrals, and conduct follow-ups as needed.
- Increase immunization coverage by providing publicly funded vaccines, including routine immunizations and annual influenza vaccines.
- Arrange appropriate visits for community members, including health assessments, referrals, and follow-up care.
- Monitor and respond to environmental health concerns in collaboration with relevant partners and agencies.
